Yizkor Memorial Form

Please enter the names of your family and friends that you would like remembered for Yizkor on Yom Kippur. Jewish custom suggests that we honor the memory of loved ones through the giving of tzedakah. Donations can be made below.

A video presentation of all the names will be available to watch on JCoOC's homepage and YouTube channel throughout the day on October 2nd.

Please submit names no later than noon on Friday, September 26th to guarantee inclusion in the video presentation and memory booklet.
